Support agencies

Home Access partners with organisations across the housing, disability, health, justice and community sectors to improve housing outcomes for people with disability.

 

Our role is to strengthen the system by:

  • sharing knowledge
  • building workforce capability
  • offering tailored guidance to help agencies navigate diverse housing pathways.

We collaborate with frontline teams, support coordinators, case managers, hospital staff, housing providers and community services to ensure people with disability receive accurate information and coordinated support.

We provide information and support to help you:

  • Explore different housing options
  • Understand your funding and support options
  • Talk through what you need in a home
  • Develop a personalised housing pathway plan
  • Connect with services that can support your housing goals

We can also link you with:

  • Financial support
  • Legal help
  • Tenancy sustainment support
  • Advocacy services
  • Peer support

Who we work with

  • housing providers and community housing organisations
  • disability support organisations and support coordinators
  • health services, including hospital and transition teams
  • Child Safety, Youth Justice and Corrective Services
  • community services and neighbourhood centres
  • advocacy organisations and cultural partners
  • agencies supporting people with disability leaving hospital, custody, or other institutional settings.

 We welcome partnerships with any organisation supporting people with disability who may need stable, safe or more suitable housing.

How we support your teams

We provide practical support to help agencies:

  • understand diverse housing pathways
  • navigate complex situations where housing and disability supports intersect
  • access up-to-date resources, tools and training
  • connect people to appropriate services or housing providers
  • build culturally safe and inclusive practices.

We do this through peer mentoring, guidance and workforce capability building across government and non-government agencies. 

We deliver:

  • information sessions and workshops
  • professional development opportunities
  • tailored capacity-building activities
  • co-working support, including case discussions and mentoring
  • updates on housing pathways, eligibility and system navigation.

Our shared aim is to strengthen sector capability so people with disability can access stable, appropriate housing that supports independence, safety and belonging.
